Medical Aid Appeal Letter

This type of appeal letter to request to grant the Medical aid benefits, or like compensatory & rewarding schemes for medical prospects would be addressed to the concerned authorities issuing such schemes. This would indicate the need to bestow with Medical aid on financial, deserving grounds etc to be briefed up.

Subject: Request to grant Medical aid as per the scheme.
Respected Sir;
In relation to the subject & with reference to the scheme for applying for Medical aid benefits; I the undersigned employee of your esteemed hospital would like to apply for the same.
Sir, enclosing herewith the photocopies of previous year’s work sheets, and birth proof etc as required & prescribed to be taken in to the consideration by the concerned committee.
Further to highlight as a deserving criterion; it is clear from my past active involvement and my past records; it seems to be quite possible to grant the scheme for Medical aid to me.
Sir, I once again kindly request to take my appeal in a positive spirit to clear some financial obstacles in the prospects.
